中心静脈栄養を実施されている患者様をはじめ、退院時から薬局の訪問が必要な患者様については積極的に退院前カンファレンスに参加しております。

患者様の状況の確認はもちろんのこと、療養環境の確認、必要薬剤や物品の確認、訪問スケジュールの作成のために必要なことと考えておりますが、退院前から関わることによって患者様に安心感を提供したいと考えております。
